ADHD Evaluation
Part 1 - $200
Focused Introductory Visit
Description:
If you’re seeking an evaluation for ADHD, this is the first step in the evaluation process.
During this focused introductory visit, you’ll meet with your provider to review your medical history, discuss your symptoms, and determine whether a comprehensive ADHD evaluation is appropriate.
Following your appointment, you’ll receive access to the Creyos cognitive assessment to complete from home.
Once you’ve completed the assessment, please allow up to 24 hours for processing before scheduling ADHD Evaluation – Part 2.
Please Note:
- This appointment does not include an ADHD diagnosis.
- Medication is not prescribed during this visit.
ADHD Evaluation
Part 2 - $160
Comprehensive Diagnostic Evaluation
Description:
Schedule this appointment only after you have completed ADHD Evaluation – Part 1 and your Creyos cognitive assessment has been processed.
During this visit, your provider will:
- Review your Creyos assessment results
- Complete your comprehensive ADHD evaluation
- Discuss whether you meet the diagnostic criteria for ADHD
- Review treatment recommendations and options, if appropriate
Please allow up to 24 hours after completing your Creyos assessment before booking this appointment.
New Patient Appointment (20 Minutes)

For All New Patients (Non-ADHD)
Description:
Choose this appointment if this is your first visit with Proper Telehealth and you are not beginning an ADHD evaluation.
This appointment is appropriate for:
- Weight Management
- Mental Health
- Primary Care
- Urgent Care
- Medication Management
- General Medical Concerns
During your visit, your provider will review your medical history, discuss your concerns, and create a personalized treatment plan tailored to your needs.
If you’re seeking an ADHD evaluation, please schedule ADHD Evaluation – Part 1 instead.
Follow-Up Appointment (10 Minutes)
$110
For Established Patients
Description:
Choose this appointment if you’ve previously been seen by a Proper Telehealth provider and are returning for ongoing care.
Follow-up appointments are appropriate for:
- ADHD medication management
- Weight management follow-ups
- Mental health follow-ups
- Primary care follow-ups
- Urgent concerns
- Medication refills
- Reviewing test results
- Treatment plan adjustments
